Provider - Epic Healthy Planet Project Manager

Position Overview

We are in search of an Epic Healthy Planet Project Manager for a U.S. client who is a big picture thinker. This senior-level individual must be able to figure out details of a Population Health strategy and create actionable plans to implement. This is a W2 hourly consulting role.

Requirements

• Bachelor’s degree or equivalent combination of education and experience
• Epic Healthy Planet certification is a plus
• Experience implementing Healthy Planet/Population Health Management, ACO reporting, or Registry databases is a plus
• Excellent customer service skills
• Be self-directed and work on collaborative teams
• Excellent communication skills, both oral and written
